Simoltaneous Emphysematous pyelonephritis and exudative pleural effusion in a 9 year old boy : An unusual pediatric case .

 Emphysematous pyelonephritis (EPN) is an acute and rapid progressive necrotizing parenchymal and prirenal infection caused by gas forming uropathogens . In literature review , EPN in children has been reported only in a 12 year old boy  with a renal transplant under treatment of immunosupressive agents and presented to hospital with high fever and elevated serum ceratinin  but we are going  to report a 9 year old boy with EPN who had neither diabetes nor immunedeficiency in past history . Unusual presentation in this case was long term period of progression without high fever , metabolic disturbances and septic appearance . Silent behaviour of this infection in this patient led to involvement of adjacent organs such as lung . This is an unusal case because of the age of presentation and clinical behaviour.
